英国の認知症介護事情を垣間みる
増加しつづける認知症患者の介護をどうするのか？介護する家族の負担が大きく、問題となっています。
介護者をどのように支援するか、という研究も行われてきています。2013年に発表された研究の序文に、英国の認知症介護事情について書かれていましたので、今回はこの序文をご紹介します。

Introduction
- In the United Kingdom, dementia care is estimated to cost £23bn per year, and this is projected to treble in the next 30 years as the number of older people increases.
- Families and individuals bear the biggest burden; two thirds of people with dementia live at home and receive most of their care from family members, who therefore save the economy a considerable amount of money.
- About 40% of carers of family members with dementia have clinically significant depression or anxiety, and others have significant psychological symptoms.
- The psychological morbidity of carers predicts a breakdown in care and therefore the need for placement in a care home as well as elder abuse.
- Thus improving the psychological health of the carers may not only improve their quality of life but also that of the recipient of their care.
英国では3.9兆円に
英国では認知症患者の介護に現状で年間約3.9兆円の費用が発生していると試算されています。今後30年でさらに増加することが見込まれています。
認知症患者の３分の２は自宅にいるため、家族の介護負担が大きくなっています。介護者の40％にうつや不安がみられている、とのことです。高齢者虐待や施設ケアへつながることがあり、どのように家族を支えていくか、模索している様子です。
このような事情は日本も似ているのではないでしょうか。
日本では10兆円？
日本でも実態調査がはじまっているようです。ヨミドクター（2014年2月11日 読売新聞）から一部引用します。
［社会的費用］家族介護の負担 金額換算 *2
この実態調査は、厚生労働省研究班の「認知症の社会的コスト推計」研究の一環だ。研究班代表を務める佐渡医師は「社会全体でどれくらいの負担があるのかを明らかにし、限られた資源を効率的に使い、家族介護の負担軽減策など必要な政策につなげていくのが狙い」と説明する。 認知症の「社会的費用」は、医療・介護費などの直接費用と、家族による無償のケアの間接費用を合算して算出する。
認知症の社会的費用については、英国や米国などで既に試算され、政策に活用されている。世界保健機関（ＷＨＯ）は１２年、「１０年時点で世界で６０４０億ドル（約６２兆円）に上る」とし、今後の急増への対応を各国に求めた。
日本では、これまでのデータから医療費は数千億円、公的介護費は６兆～７兆円程度。家族介護などの間接費用を既に試算している英国並みと仮定すると、全体で１０兆円を超す可能性がある。

「わが国における認知症の経済的影響に関する研究」へのご協力のお願い
社会的にも大きな問題です。家族介護の負担をどのように軽減していくか、これからの高齢者サービス全体像を踏まえて、考えていかなければならないでしょう。
