進行がんにビタミンC大量静注療法は有効ですか?

 進行したがんにビタミンC大量静注療法が有効とネットに書いてありましたが、本当ですか?

都市伝説でしょうか

 この質問、しばしば耳にするのですが、有名な民間療法でもあるのでしょうか。都市伝説なのでしょうか?

 ビタミンCで少しでも効果が出る、というのなら、副作用も少ないでしょうから、ぜにやってみたいと思いますが。まあ、そう簡単ではないでしょう。

 

 ちなみに、ビタミンC(アスコルビン酸)注射液の効能はどのようになっているでしょうか。

効能又は効果
◇ビタミンC欠乏症の予防及び治療(壊血病,メルレル・バロー病)
◇ビタミンCの需要が増大し,食事からの摂取が不十分な際の補給(消耗性疾患,妊産婦,授乳婦,はげしい肉体労働時など)
◇下記疾患のうち,ビタミンCの欠乏又は代謝障害が関与すると推定される場合 
・毛細管出血(鼻出血,歯肉出血,血尿など)
・薬物中毒
・副腎皮質機能障害
・骨折時の骨基質形成・骨癒合促進
・肝斑・雀卵斑・炎症後の色素沈着
・光線過敏性皮膚炎

上記の諸症のうちビタミンC欠乏症の予防及び治療,ビタミンCの需要が増大し,食事からの摂取が不十分な際の補給以外の効能・効果に対しては,効果がないのに月余にわたって漫然と使用すべきでない。

 

 がんの終末期で食事が十分とれない場合には、消耗性疾患ということで保険適用されるのでしょうか。

 「激しい肉体労働時」でも使えるとは、知りませんでした。食事がとれないのに、ビタミンCを注射しながら激しい肉体労働する、というのは、いつの時代の想定でしょうか?

 ともかく、食事からの摂取が不十分な場合には、治療の選択肢となっています。

 

 では実際の効果はどうでしょうか?

 PubMed Clinical Queriesで調べてみたところ、いくつかヒットしました。

ランダム化比較試験(Creagan, 1979年) *1

The two groups showed no appreciable difference in changes in symptoms, performance status, appetite or weight. The median survival for all patients was about seven weeks, and the survival curves essentially overlapped.

ランダム化比較試験(Moertel, 1985年) *2

Vitamin C therapy showed no advantage over placebo therapy with regard to either the interval between the beginning of treatment and disease progression or patient survival.

総説(Wilson, 2014年) *3

Currently, the use of high-dose IV vitamin C cannot be recommended outside of a clinical trial.

 

どれも無効 

 ざっと見たところ、ビタミンC大量静注療法はどれも無効、推奨しない、といった結論となっています。

 

 Wilsonらの2014年の総説は、研究の全体像を眺めるのによいかもしれません。少し本文を確認してみましょう。

 論文はpdfにて入手可能です。臨床研究の結果については、p.25以降に書かれています。

 

効果がみられたのはランダム化でない研究のみ

 Figure 2 が参考となるでしょう。

 ランダム化されていないCameronら、Murataらの研究では生存期間の大幅な延長がみられていますが、ランダム化比較試験のCreaganら、Tschetterら、Moertelらの研究では生存期間にほとんど差がない、という結果です。

 

 本文ではこのように解説されています。

Cameron and Pauling then published two historically controlled trials, each comparing 100 patients treated with high-dose vitamin C, with 1000 controls matched by age, sex, tumor site and histological features (Fig. 2). Both trials found a significant prolongation of mean survival (210 vs 50 days and 293 vs 39 days, respectively). However, neither of these trials was standardized by two critical prognostic factors: performance status and stage. The consistency of determination of “untreatability” is also controversial in the initial trial: 20% of the control group died within a few days of being deemed untreatable compared with none in the treatment group.

Cameron and Campbell published the only trial that measured plasma levels in association with survival time and demonstrated a linear relationship between dose and IV plasma vitamin C levels. Levels above 3 mg/dL (0.17 mM) were reported as desirable but this concentration based on recent literature appears too low to exert a pro-oxidant effect. Treatment was not randomized and was dependent on clinician preference, creating the potential for selection bias. Again there was no stratification by performance status.

 

 ランダム化されていないために、予後が悪そうな人を対照群に割り当てた可能性が高い、と指摘しているわけです。

 対照群の20%が数日以内で死亡するなど、恣意的だったのでしょう。

 引用されていた日本人の論文については特にコメントがありませんでしたが、同じような理由でしょう。

 

 ランダム化比較試験はメイヨークリニックの研究のようですが、ランダム化することで、両群を比較する上での不公平さ(選択バイアス)はなくなります。

 その結果がプラセボと有意差なし、というのであれば、説得力があります。

 

 ということで、歴史的に効果あり、と結論した研究はバイアスであった可能性があります。

 

これからどうするか?

 これまで通り、ビタミンC大量静注療法は効果がないことを伝えるでしょう。また、実施の際には、保険適用されるかどうかについては十分な考慮が必要です。
