音楽を聞くことはどんな効果があるのか

 以前、音楽療法に関する医学論文を紹介したことがあります。このような論文の多くは、音楽療法士などが提供する、専門的治療としての音楽療法となっています。

 

 しかし、音楽療法士による音楽療法を受けることは、現時点ではあまり簡単ではありません。

 例えば、病院で受けようとしたとしても、簡単ではないでしょう。サービスへのアクセスしやすさという観点では、日本国内ではまだまだこれから、というところです。

 

 最近ではポータブルデバイスやスマートフォンも普及し、音楽はだれにでも手軽に楽しめるものとなっています。

 そこで、音楽を聞くことにはどんな効果があるのか、について調べてみました。

 

認知症に対する音楽療法の効果

 認知症に対する音楽療法の効果については、2016年のメタ分析があることは以前ご紹介しました。

 音楽療法によって認知機能改善は望めないこと、破壊的行動が少なくなる可能性があること、認知症のうつや不安が改善する可能性があることなどが、これまでの研究の知見から明らかになりつつあります。

 

音楽を聞くことの効果

 それでは、音楽を聞くことについてはどうでしょうか。2015年にランダム化比較試験が発表されていますので、確認しておきましょう。

研究の概要

 周辺症状(BPSD)のある中等度から重度の認知症高齢者(施設居住者)に音楽療法または音楽鑑賞(週2回、20セッション)を行うと、標準ケアに比べて行動・心理症状 *2 は改善するか、を検討したランダム化比較試験。

 

主な結果

 120人を40人ずつ3群にランダム割付。治療終了時点での評価は98人(音楽療法群 31人、音楽鑑賞群 32人、標準ケア群 35人)。

 治療終了時点でのThe NPI global score(0-144点)は、音楽療法群では 28%、音楽鑑賞群では12%、標準ケア群では21%改善がみられ、有意差はなかった。妄想、不安、脱抑制のサブスケールでも有意差はみられなかった。

 

 認知症の周辺症状(BPSD)*3に対する効果を検証した、小規模ながら質の高い研究です。

 本研究の追跡率は82%で、結論が逆転する可能性を孕んだ脱落数です。まあ、脱落が多くなるのは対象者の性質上(病状悪化や死亡などによる脱落が多い)やむを得ないところですが。

 

 いずれにせよ、認知症のBPSDに対して音楽鑑賞群ばかりか、音楽療法群でも効果がみられなかった、という結論でした。

 

この研究ではどんな介入が行われたのか

 この研究ではどのような介入が行われたのでしょうか。論文を確認しておきましょう。

 まずは音楽療法群から。PWDはpersons with dementiaの略です。

Music Therapy

Participants randomized to music therapy participated in 20 individualized 30-minute sessions, twice a week for 10 weeks. A certified, specifically trained music therapist conducted all sessions, which took place in a quiet, medium- sized room. The site and the musical instruments available did not change during the treatment period. PWDs were presented with a similar group of instruments at each session and encouraged to pick them up and interact with them. The music therapist followed the PWDs’ rhythm and music production (also introducing variations) to create nonverbal communication. During the session, the music therapist built a relationship with the PWD by singing and using melodic and rhythmic instruments (improvisation), facilitating the expression and modulation of the PWD’s emotions and promoting “affect attunement” moments. All sessions were videotaped using a fixed camcorder to perform a music therapy assessment.

Listening to Music

PWDs randomized to LtM underwent 20 individualized 30-minute sessions, twice a week for 10 weeks. During the session, the PWD listened to music from a preferred playlist without any interaction with a music therapist or formal caregiver. The PWDs did not wear earphones and remained in their rooms or in a quiet, private place during the session. The music therapist had created the playlists on the basis of interviews with the PWD (whenever possible) and formal or informal caregivers.

 

 本人からの聴取をもとに音楽療法士が作成したプレイリストから聞いてもらう、という30分のセッションを週2回10週間、合計20セッション行われています。

 鑑賞には音楽療法士や介護者が関わらず、静かな部屋でイヤホンをつけないで鑑賞してもらっています。

 

 音楽を聞くことの効果を検証するには、対象者の理解力、脱落、効果の測定(アウトカムの評価)など難しさがあるでしょう。

 全く別の方法論や評価軸が必要なのかもしれません。
