 子宮頸がん検診には効果がありますか?(2) - 地域医療日誌につづきます。

ACPガイドライン

 検診の効果はどの程度かということと、国として検診を推奨するかどうかということはまったく別です。一定の効果がある検診であっても、予算がなければ公費で実施することは困難でしょう。

 検診は健康な人に対する予防になりますから、いかに費用や害を少なくして最大限の効果を上げるか、十分な吟味が必要です。

 ガイドラインでは、一般的にそのような検討をした上で、推奨される方法を示すものとされています。

 

 ちょうどこの原稿を書いている最中、子宮頸がん検診に関するガイドラインが米国内科学会(ACP)から発表されました。何といういいタイミングでしょう。

 ACPはエビデンスをよく吟味していますので、HPV検査がどのように記載されているかについても確認してみましょう。

診療ガイドライン(Sawaya, 2015年) *1

BEST PRACTICE ADVICE 1: Clinicians should not screen average-risk women younger than 21 years for cervical cancer.

BEST PRACTICE ADVICE 2: Clinicians should start screening average-risk women for cervical cancer at age 21 years once every 3 years with cytology (cytologic tests without human papillomavirus [HPV] tests).

BEST PRACTICE ADVICE 3: Clinicians should not screen average-risk women for cervical cancer with cytology more often than once every 3 years.

BEST PRACTICE ADVICE 4: Clinicians may use a combination of cytology and HPV testing once every 5 years in average-risk women aged 30 years or older who prefer screening less often than every 3 years.

BEST PRACTICE ADVICE 5: Clinicians should not perform HPV testing in average-risk women younger than 30 years.

BEST PRACTICE ADVICE 6: Clinicians should stop screening average-risk women older than 65 years for cervical cancer if they have had 3 consecutive negative cytology results or 2 consecutive negative cytology plus HPV test results within 10 years, with the most recent test performed within 5 years. 

BEST PRACTICE ADVICE 7: Clinicians should not screen average-risk women of any age for cervical cancer if they have had a hysterectomy with removal of the cervix.

 

 一般的なリスクの女性に対する検診について簡単にまとめると、

  • 21歳未満:子宮頸がん検診をすべきではない。
  • 21歳以上29歳未満:3年に1回の細胞診を。
  • 30歳以上:5年に1回の細胞診とHPV検査を組み合わせてもよいかも。
  • 65歳以上:過去の検査結果に応じて検診を終了。

 ということになるでしょう。

 HPV検査は30歳以上で推奨されており、30歳未満では実施すべきではない、とされています。

 理由としては、このように書かれています。

 Because the prevalence of highrisk HPV infection peaks shortly after initiation of sexual intercourse, rates of positive HPV test results are highest among women younger than 25 years and decrease with advancing age.

 Positive results occur in
about 30% of women aged 21 to 24 years compared with
about 12% of those aged 30 to 34 years and
5% of those aged 60 to 64 years.

 Rates of cytologic abnormalities also decrease with age:
About 13% of women aged 21 to 24 years have abnormalities compared with
about 7% of those aged 30 to 34 years and
3% of those aged 60 to 64 years.

 In the setting of a prepaid health plan, about 9% of women aged 30 to 64 years had either an abnormal cytologic test result or a positive HPV test result.

 

 21~24歳では約30%がHPV検査陽性となってしまうため、という理由でした。また、30歳以上においても、細胞診とHPV検査の組み合わせでは約9%が陽性となってしまう、と指摘しています。かなり高い陽性率のようです。

 検診間隔をどの程度にすべきかについても検討されており、検診の害や過剰使用についての記載がガイドラインにも目立ちます。

 

 米国FDAは2014年、ハイリスクHPV検査のみを検診検査として認可しており、専門家の暫定ガイダンス *2 として、25歳からのHPV検査開始を推奨する動きもあるようです。この動きに対して、このように牽制しています。

In 2014, the U.S. Food and Drug Administration approved high-risk HPV testing alone as a primary screening test, and recent interim guidance from a group of experts recommended triennial screening beginning at age 25 years. This decision will be controversial given current recommendations to avoid HPV testing in women younger than 30 years, due in part to the relatively high prevalence of HPV among women aged 25 to 29 years (21%).

 

 25~29歳ではCIN3+(前がん病変)の約30%を占めており、頻度が高まるため検査したほうがよいとする意見と、30歳未満では21%とHPV検査陽性率が高くなるため見送ったほうがよいという意見と、議論は分かれているようです。

 妥当な落とし所が見つかるのか、米国では議論の真っ只中というところでしょう。これからも注目しておきたいテーマです。

 

子宮頸がん検診には効果がありますか?

  • 子宮頸がん検診を検討した質の高いランダム化比較試験はひとつのみ。
  • 子宮頸がん検診による総死亡の検討はいまだなされていない。
  • ランダム化比較試験では子宮頸がん検診を行うと子宮頸がん死亡が35%少ない。
  • 子宮頸がん検診のうち、HPV検査では最も子宮頸がん死亡の予防効果が高い。
  • HPV検査は30歳未満での陽性率が高いため、導入には慎重な意見がある。
