麻しんの接触後予防は72時間以内に

 

MRワクチンがワクチン品薄に

 麻しん風しんワクチンが品薄状態となっています。

 流行がニュースになると、ワクチン希望者が医療機関に押し寄せ、さらに医療機関もワクチンを必要以上に確保するため、ワクチンの供給ができなくなるという、まるで発展途上国のような事態が頻発しています。

 数年前の風しん流行時にも、似たようなことがありました。

 

 流行してからでは、大抵手遅れです。

 報道でパニックに陥るのではなく、日頃からワクチンに関心をもち、流行に備えておきたいものです。

 

麻しんのPEP

 接触後のワクチンでも予防できますか? - 地域医療日誌 でも紹介しましたが、麻しん患者に接触した場合、72時間以内にワクチン接種をすると発症予防が期待できます。

 この論拠となる文献がはっきりしませんが、アメリカ疾病予防管理センター(CDC)のガイドラインに、引用文献なしで記載があります。

Post-exposure Prophylaxis

People exposed to measles who cannot readily show that they have evidence of immunity against measles should be offered post-exposure prophylaxis (PEP) or be excluded from the setting (school, hospital, childcare). MMR vaccine, if administered within 72 hours of initial measles exposure, or immunoglobulin (IG), if administered within six days of exposure, may provide some protection or modify the clinical course of disease.

MMR vaccine as post-exposure prophylaxis

If MMR vaccine is not administered within 72 hours of exposure as PEP, MMR vaccine should still be offered at any interval following exposure to the disease in order to offer protection from future exposures. People who receive MMR vaccine or IG as PEP should be monitored for signs and symptoms consistent with measles for at least one incubation period. If many measles cases are occurring among infants younger than 12 months of age, measles vaccination of infants as young as 6 months of age may be used as an outbreak control measure. Note that children vaccinated before their first birthday should be revaccinated when they are 12 through 15 months old and again when they are 4 through 6 years of age. Except in healthcare settings, unvaccinated people who receive their first dose of MMR vaccine within 72 hours after exposure may return to childcare, school, or work.

 

 何らかの発症予防効果や臨床経過の改善が見込まれるかもしれない、といった論調です。実際にこの効果を検証することは難しいでしょう。

 

接触した免疫のない医療従事者には長期休暇

 さらに、このあとに気になることが書かれています。

Post-exposure prophylaxis for healthcare personnel If a healthcare provider without evidence of immunity is exposed to measles, MMR vaccine should be given within 72 hours, or IG should be given within 6 days when available. Exclude healthcare personnel without evidence of immunity from duty from day 5 after first exposure to day 21 after last exposure, regardless of post-exposure vaccine. 

 

 免疫が確認できない医療従事者が麻しん患者に接触した場合、接触後予防接種の有無に関わらず、最初に接触した日の5日後から、最後に接触した日の21日後まで仕事からはずすこと、となっています。

 なかなか厳しいですね。
